Choose a defensible starting point for a fuller maths baseline with a six-domain SEN placement screen.

What is included?
A 45-page A4 landscape PDF, plus a separate four-page preview: 24 pupil checkpoint pages with two tasks per page (48 evidence points), four administration pages, six adult key pages, six domain records, two profile/next-step pages, cover and selective-print contents.

Four provisional routes
A: Early KS1; B: KS1; C: Early KS2, including Years 3–4 content; D: KS2, including later KS2 demands. The six domains are number/place value, addition/subtraction, multiplication/division, fractions, measurement/functional maths and geometry/statistics. Two items cannot establish mastery of a key stage.

Prepare and administer
Use selected checkpoints in short, untimed sessions. Provide blank working paper and usual recording tools. Neutral counters are allowed only for the tasks identified in the guide, including an exact eight-counter sharing script. Pointing, AAC, speech and exact scribing are accepted when they preserve the target. For the numeral-reading task, the adult must not first read the numeral aloud. Keep keys hidden and record any later teaching separately.

Incomplete and split profiles
Score items 1, 0 or NA and record each domain route. With fewer than five valid domain routes, do not finalise an overall route. With five, an overall route is provisional and names the missing domain. A spread of two or more route steps calls for domain-specific sampling. For coherent adjacent routes, use the most frequent; a tie starts at the lower route while retaining higher strengths. Stop for distress, fatigue or invalid access.

Print selected pages at Actual size / 100%, one PDF page per A4 landscape sheet. Pupil directions, options and diagram labels are at least 16 pt; numerals are larger where space permits. Small branding and footers are not pupil task text. Green identifies Assessment & Progress; level names are written as well as colour coded. The PDF has fixed pages, not editable worksheets; physical materials are not included.

Designed for teacher-led SEN/SEND assessment, including older pupils working on earlier mathematical content. Select by current skills and access needs, not chronological age. This is a teacher-designed curriculum sample, not a standardised test, diagnosis or certificate of whole-stage attainment.
