Free printable phonics assessment for Phases 2-5: grapheme checks, real and nonsense word reading, tricky words, a session log and parent summary.

A simple way to check and record a pupil’s phonics: where they started, and what has changed since. Designed for tutors and TAs working one to one.

What’s included (5 A4 pages)

  • Grapheme checks for Phases 2, 3 and 5. Point to each grapheme and record the response, with separate First check and Latest check columns so a starting point is never mistaken for progress.
  • Selected alternative pronunciations (e.g. ow in cow and snow, ea in sea and head), each word recorded separately.
  • Tricky words: write in the words from your pupil’s own programme.
  • Reading words: real and nonsense words for each phase, including Phase 4 adjacent consonants, following the format of the Year 1 phonics screening check.
  • Session record and a progress summary for parents.

How it works

Mark what happened in each check: ✓ correct without help, P correct after a prompt, – not yet. Leave a box blank if you didn’t check it. Clear instructions cover two-sound graphemes (th, oo), accents and scoring nonsense words.

Good to know

  • An informal check, not a formal or screening-check assessment, and it has no pass mark.
  • Uses the original Letters and Sounds phase groupings. These do not map directly to other programmes (e.g. Little Wandle, Read Write Inc.), so check only what your pupil has been taught and follow their school’s programme.
  • Set in Andika, a font designed for early readers, with the single-storey a and g children learn to write.
  • Reviewed by an educator before publication.
