An AI prompt that drafts the transition section of a US IEP: measurable postsecondary goals, annual goals, and a transition services table, ready to edit.

Writing the transition section from scratch for every student on a caseload takes hours. I built this prompt for my own transition IEPs. You fill in a short student profile, paste it into the AI tool your district allows, and get a solid first draft to edit with your team.

What the prompt gives you back (10 labeled sections):

  • A PII check, so the AI flags anything identifying before it writes
  • A transition assessment summary
  • Measurable postsecondary goals for education/training, employment, and independent living
  • 2 or 3 annual IEP goals, each tied to a postsecondary goal
  • A transition services table (activity, type, who, when)
  • Course of study, agency questions, and age-based timeline flags (federal age 16, Kansas age 14, age of majority)
  • Questions for your IEP team, with (TEAM INPUT NEEDED) marked anywhere the AI won鈥檛 guess

What鈥檚 inside:

  • The copy-ready prompt, as a PDF page and a plain text file that copies cleanly
  • How to use it with ChatGPT, Claude, Grok, or Gemini
  • A NO STUDENT PII guide: what to leave out, plus quick de-identify swaps
  • A fill-in Student Profile Worksheet that matches the prompt鈥檚 brackets
  • A Weak vs. Strong input guide
  • 2 sample outputs (fictional students) so you can see what a first draft looks like. The prompt, worksheets, and checklists are written by me; the samples are AI drafts, shown unedited.
  • A review checklist for AI drafts before anything goes into an IEP

Great for:

  • Case managers and transition coordinators with big caseloads
  • 18 to 21 program teachers
  • New SPED teachers learning what a strong transition plan looks like
  • Prepping a draft before the IEP meeting

Please note: never enter student names or identifying information. Follow your district鈥檚 AI and data privacy policy. Outputs are drafts. The IEP team makes every decision. This is not legal advice.

Written by a transition coordinator with AI assistance; outputs are drafts for the IEP team.

Pair it with my IEP Progress Report Writer AI Prompt and the free Transition IEP Self-Check.
