ABC data sheets and behaviour observation forms for SENCOs, teachers and teaching assistants in KS1 to KS4: clear, ready-to-print forms to observe, record and summarise pupil behaviour before a support plan, review or team meeting. Thirty-one pages, supplied in colour and in ink-friendly black and white.

Use these ABC data sheets to record what happened before a behaviour (antecedent), what the pupil did (behaviour) and what followed (consequence), then look for patterns across a week or two. The forms stay neutral and factual, with prompts to describe only what you can see or hear and to protect pupil privacy.

What is included (31 pages in two versions, 9 PDF files)

  • 2 adult guides and an “observable behaviourâ€ page
  • ABC basics, narrative ABC form, checklist ABC form with a worked example, and ABC for two behaviours
  • 2-week log
  • Frequency forms
  • Interval recording guide, with 5-minute and 10-minute interval sheets
  • Duration and latency forms
  • Antecedents and setting events
  • Pupil questionnaire (illustrated) and adult questionnaire
  • Data summary with graph, trends and next steps
  • Team meeting notes and a family communication page

The two versions have the same content. Each is split into parts (4 black and white, 5 colour) for easy download.

Ways to use

  • SENCO observations before an SEN support review
  • Behaviour tracking for a pupil with additional needs
  • Evidence for pastoral and multi-agency meetings
  • Training teaching assistants to record observations consistently

These forms help teams observe and support pupils; they do not replace an assessment by a qualified professional. Examples use a fictional pupil.

PDF, US Letter (choose “Fit to page to print on A4). English.
